Care Options & Care Management

We assist persons 60 and older on how to best pair personal resources with community-based services. Services range from daily living assistance to comprehensive targeted care management.

EnhancedCare Solutions

Specialized emotional, physical, and safety support. This program coordinates targeted home-based services to protect overall well-being, including In-Home Counseling for anxiety and depression, care coordination via the GUIDE Model for dementia, and victim advocacy through Safe Haven for elder abuse.

Primary Care at Home

We provide home-based medical assessments and comprehensive care management for older adults and individuals with disabilities who face barriers leaving home.

In-Home Counseling

Access personalized, professional in-home and virtual counseling services designed to help home-limited adults navigate anxiety, depression, and loneliness.

GUIDE Model

Discover how the GUIDE Model supports individuals with dementia and their caregivers through personalized care coordination, training, and respite services to help patients remain safely at home.

Safe Haven

Safe Haven provides confidential support, advocacy, and care management for older and vulnerable adults experiencing abuse, neglect, or financial exploitation.

Community Care Closet

Providing essential consumable items, such as liquid nutritional supplements and adult incontinence supplies, to community members in need through generous local donations.

Long Term Care Ombudsman

Advocates for residents of licensed long-term care facilities. Ombudsmen help residents understand their rights, resolve complaints, and work to improve their quality of life.

Caregiver Support

Providing support and resources to those caring for loved ones. Our trained Support Specialists give advice, tips, guidance, and offer an outlet to share concerns so they won’t be alone in their caregiver journey.

MI Choice Waiver Program

MI Choice Waiver is a long-term care program offered to older adults and adults living with a disability who medically require and financially qualify to receive a higher level of care. Through MI Choice, adults receive Medicaid-covered care services in the home.

Medicare Help (SHIP)

Managed through the state’s sixteen Area Agencies on Aging, SHIP is designed to educate, counsel, and empower older adults and individuals with disabilities to make informed benefit decisions. The program is free, unbiased and staffed with trained volunteers.
