Care Options & Care Management
Care programs provide options that enable individuals to remain independent while living in their own home, with family, or wherever they choose. Services like adult day care, homemaking, respite, personal care, and medication management are provided in partnership with our extensive network of screened partners. Services are given both in or out of the home. EnhancedCare Solutions services, Primary Care at Home and In-Home Counseling, may also be included with the managed care programs.

Case Coordination and Support
This program supports individuals who are experiencing challenges with everyday activities but do not require extensive supervision or nursing care. The goal is to help participants maintain their independence and safely remain in their homes. Funding is provided in part through the Older Americans Act and the Older Michiganians Act. Participants may choose to make a voluntary contribution to help support the cost of services.
Care Management
This program serves individuals who require a greater level of assistance and support to remain safely at home and who are not eligible for Medicaid. A Support Coordinator and Registered Nurse work together to assess each individual’s needs and develop a plan for appropriate in-home services. Services are funded in part through federal and state resources provided under the Older Americans Act and the Older Michiganians Act. Participants may choose to make a voluntary contribution to help support the cost of services.
Note: We can help you determine the level of care required with an in-home assessment, coordinate services with area providers.
Targeted Care Management
The Targeted Care Management program serves Medicaid-eligible individuals who require a higher level of care and support and are not enrolled in the MI Choice Waiver Program. Support Coordinators provide comprehensive assessment, person-centered care planning, service coordination, ongoing monitoring, and periodic reassessments to help individuals remain safely and independently in their homes and communities. Services are funded through a combination of the Older Americans Act, Older Michiganians Act, and the Michigan Department of Health and Human Services Independent Living Program.
Veteran Directed Care
Veteran Directed Care (VDC) empowers eligible Veterans to direct their own long-term services and supports, giving them greater flexibility, choice, and control over their care. Senior Resources provides care management services for Veterans enrolled in the Veteran Directed Care program. Following referral by the VA, our care managers work with Veterans and their caregivers to develop individualized care plans and coordinate supports that promote independence, dignity, and quality of life.
Be Aware of Scams
There are a variety of scam telephone calls directed at older adults that impersonate Medicare, Medicaid, Social Security and benefit programs. DO NOT give personal information to these callers. Please report these incidences to the State Health Insurance Assistance Program office at (231) 733-3572, or the Office of Inspector General Fraud Hotline at 1 (800) 447-8477.
Who's Eligible?
- Adults age 60+
- Adults age 65+ or age 18-64 and certified disabled for the MI Choice Waiver Program
- Needs assistance with daily living tasks to remain independent
- Reside in Muskegon, Oceana or Ottawa County
What's the Process?

- Complete an initial care review with an Options Counselor
- Provide name, address, birth date, marital status, medical, and income information
- Conduct an income and asset information review with an Options Counselor
- Care options are reviewed and an initial assessment may be scheduled
