Care Options and Resources
Care options vary and depend on the level of care needed and how that care is delivered and funded. To help navigate these options, our trained Options Counselors provide guidance to identify the services that may be helpful. This free service is conducted over the phone or in person.

Care Program Options
Care programs provide options that enable individuals to remain independent while living in their own home, with family, or wherever they choose. Services like adult day care, homemaking, respite, personal care, and medication management are provided in partnership with our extensive network of screened partners. Services are given both in or out of the home. Primary Care at Home, Senior Resources’ home-based medical care, may also be included with managed care programs.
Who’s Eligible?
- Adults age 60+
- Adults age 65+ or age 18-64 and certified disabled for the MI Choice Waiver Program
- Needs assistance with daily living tasks to remain independent
- Reside in Muskegon, Oceana or Ottawa County
What’s the Process?
- Contact an Options Counselor at 231-733-3585 or submit a secure, HIPAA-compliant referral form to initiate contact from an Options Counselor
- Complete an initial care review with an Options Counselor
- Provide name, address, birth date, marital status, medical, and income information
- Conduct an income and asset information review with an Options Counselor
- Care options are reviewed and an initial assessment may be scheduled
Care Program Options
Case Coordination and Support
Individuals not requiring a higher level of supervision or care but experiencing difficulty with daily living activities; allows them to stay safely in the home. Funded in part by federal and state funds allocated under the Older Americans Act and Older Michiganians Act.
Care Management
Individuals requiring a higher level of supervision or care and are not Medicaid eligible. A Supports Coordinator and Registered Nurse assess need and arrange for in-home services.
Funds are restricted. Participants may be limited to services only once or twice per week. Participants asked to cost-share if able. Funded in part by federal and state funds allocated under the Older Americans Act and Older Michiganians Act.
**If you are able to pay for services, we can conduct an in-home assessment, determine what level of care is required, coordinate services with area providers and bundle the services in one monthly invoice.
Targeted Care Management
Individuals requiring a higher level of supervision or care that are Medicaid eligible and not enrolled in the MI Choice Waiver program. Supports Coordinators offer assessment services, care planning, service coordination, follow-up and re-assessment.
Funded through a combination of Older Americans Act, Older Michiganians Act, and Department of Health and Human Services’ Independent Living Program.
MI Choice Waiver
Available to individuals that are Medicaid eligible that meet medical, income, and asset criteria.
Options Counselors will also provide information on other programs such as Veterans Administration Services (VA), LifeCircles PACE (Program of All-Inclusive Care for the Elderly) and other programs that will help an older adult live safely and independently at home as well as housing options from assisted living to nursing facilities in our service area.
