Senior Resources Provider Partners

Purchase of Service Partners Network
Senior Resources contracts with local businesses to provide various funded services to serve participants in the MI Choice Waiver and Care Management programs. Partners must meet a defined quality standard established for each service area and include a person-centered model of service delivery. Currently, we have Purchase of Service (PoS) Agreements with over 100 businesses. The services purchased most often include:
- Chore Services
- Durable Medical Equipment Providers
- Adult Day Care Services
- Home Delivered Meals
- Personal Emergency Response Providers
Those interested in potentially joining our network of service providers should contact our Contracted Services Coordinator at 231-733-3524, 231-739-5858, or 1-800-442-0054 or [email protected]
