As one of sixteen Area Agencies on Aging in the state of Michigan, we focus on supporting the independence, dignity, and well-being of older adults and adults with disabilities in Muskegon, Oceana and Ottawa counties. Through program planning and an in-depth knowledge of the needs of individuals in our service area, we offer unbiased care options allowing them to make informed choices.

We assist all persons 60 and older on how to best pair personal resources with home and community-based services funded in part by the Older Americans Act, Older Michiganians Act, and local resources. Services can include assistance with daily living activities to comprehensive targeted care management services.

We’ve Revived the House Call

EnhancedCare Solutions

We offer home-based services for older adults and adults living with a disability that find it challenging to leave home—a solution for their physical health and emotional well-being.

Primary Care at Home medical staff provide a wide-range of services that help reduce hospitalizations and emergency room visits.

In-Home Counseling connects caring professionals to help participants live healthier and be more hopeful.
